As a participant in the above-referenced event (hereinafter the “Event”), I acknowledge the ongoing contagious nature of the Novel Coronavirus, COVID-19, and its variants. I further acknowledge that both the United States Centers for Disease Control (“CDC”) and the New York Department of Health (“NYSDH”) continue to recommend that certain precautions be taken outside the home.
I further acknowledge that The New York State Society of Certified Public Accountants, as well as its chapters and affiliates (collectively, the “NYSSCPA”), have put reasonable measures in place to reduce the likelihood of spread of the Coronavirus. Notwithstanding that fact, I understand that the NYSSCPA cannot guarantee that I will not be exposed to the Coronavirus in connection with my participation in the Event.
*Participants should be aware that, as of the date this form was created, New York City generally requires that participants be fully vaccinated for events held within city limits.
With full knowledge of the risks involved, I hereby release and agree to hold the NYSSCPA harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any loss, damage, injury, or death, that may be sustained by me related to COVID-19. I understand that this release discharges the NYSSCPA from any liability or claim that I, my heirs, or any personal representatives may have against the NYSSCPA with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, the Event. This liability waiver and release extends to the NYSSCPA together with its affiliates, directors, trustees, officers, members, employees, contractors, agents, representatives, successors, and assigns. If I am completing and submitting this form electronically, I agree that such submission constitute my signature hereto.
